To expedite your loan process, please click the following loan application form and type your information. Please double check before printing to make sure it is free of errors. After printing, please sign and attach all of the following documents and send them to Sam for processing. We usually respond you within the next business day.  Please click the following link, and print the loan application, or fill it out. It is the most recent version of loan application in which you can type your information and then print it. You should hand it over to Sam personally with the following documents.

  1. 2 years of W-2s from employment (Required)

  2. 2 years of 1040 tax return (if self employed)

  3. Copies of pay stubs equivalent of 1 month consecutive pay periods, the last and the most recent 2 pay stubs. (Required)

  4. Copies of 2 months bank statement (Checking/Saving) (Required)

  5. Copies of last 2 years K1 tax return (if applicable)

  6. Copies of stock portfolio statement (if applicable)

  7. Copies of 401k or other retirement plan statements (if applicable)

  8. Copies of current mortgage statement for 1st and 2nd liens

  9. Name and address of current landlord (if applicable)

  10. Credit Report fee of $30.
